Prescription for Chest X-Ray

Chest x-ray performed at a nearby Imaging Center for an additional cost ranging between $35-60. Applicant must provide one of the following to receive a prescription:

1. Previous positive Tb skin test
2. Previous positive Tb blood test
3. Previous chest x-ray result for Tb
